What APCM is
Advanced Primary Care Management is a CMS Medicare program that launched January 1, 2025. It pays a set amount every month for each enrolled patient whose ongoing care your practice manages - turning care coordination into predictable, recurring revenue.

Because the payment recurs every month for every enrolled patient, the numbers add up: 200 patients at the G0557 rate come to about $129,000 a year before geographic adjustment. Build your own number on traditional Medicare patients rather than Medicare Advantage, leave out anyone already on CCM, and treat any eligibility count as a floor.
Who qualifies
APCM is open to any primary care practice that bills Medicare - from solo and two-provider practices to larger groups and FQHCs, and across MDs, DOs, NPs, and PAs.
